We’re Convenient, Professional and Affordable!

If you’re like most teens, you feel great most of the time. Occasionally, small health problems occur and these problems can be warning signs of a temporary illness or a serious disease. Sometimes illnesses can be caused by problems like poor diet or too much stress-conditions that can damage your health for life.

Mission

The Henry Ford Health System School-Based Health Initiative believes that both mental and physical health are essential for a student’s success. Our health centers strive to provide comprehensive, interdisciplinary, and confidential health services that will meet the needs of students in an atmosphere that is caring and provides mutual respect.

Access to Services

Services are available to youth ages 10 years thru 21 years old.

If you are under 18 years of age, a consent form must be signed by a parent or guardian.  The consent form must be updated annually with updated health history and insurance information.  A copy of the student’s vaccine record also needs to be submitted and will be entered into MCIR, the Michigan state vaccine computer registry.

Emergency Care, Crisis Intervention, and Michigan Minor Law criteria exemptions do not require parental consent.

Payment

All services are confidential and are offered with no out-of-pocket cost.

All students are eligible to receive health care regardless of their insurance situation.  The health center accepts all insurances.  Third party insurance will be billed for appropriate services.

However, if the student has no health insurance the Fitzgerald Health Center can assist with an online Medicaid application to determine eligibility and obtain insurance, if eligible.  At this time, there are no out-of-pocket expenses billed to the student for services.

Community Advisory Committee

The Fitzgerald Health Center would like to invite you to join our Community Advisory Committee.  The goal of the committee is to have representation from parents, students, teachers, and community members.  Meetings are held at least twice per year.

The purpose of the Community Advisory Committee is to work in collaboration with the Henry Ford Health System School Based and Community Health Program to:

  • Gain and maintain the support of the community for the health center
  • Approve applicable policies annually
  • Make recommendations for non-clinical policies and activities
  • Assist in defining future goals for the health center
  • Assist in recruiting patients for the health center
  • Assist in marketing the health center to the surrounding community
  • Engage with youth to ensure youth involvement
